Is this course suitable for me?

This module is aimed at health care professionals working in a variety of health care settings who wish to advance their practice in working with patients who suffer from Long Term Conditions and requires rehabilitation.

Overview

The aim of the module is to provide critical insight into the principles underpinning the management and rehabilitation of people with long term conditions and rehabilitation needs. You will be afforded opportunities to critically appraise the National Service Framework aimed at those with long term conditions and enable them to apply theoretical knowledge to your area of practice.

Content

This module will explore;

  • Rehabilitation: case management theory, allied professionals, social work, exercise, nutritional aspects, telemedicine/health, hospital at home, rehabilitation in the community, interpersonal issues.
  • Principles of rehabilitation: neurological, cardiac, pulmonary rehabilitation and rehabilitation of patients with recurring cancers for students working with clients; management of symptoms.
  • Management of chronic diseases, diabetes, rheumatoid/osteo arthritis, mental health, renal diseases.
  • Psychosocial issues: adherence, coping, mood and relationship.
  • Frameworks/models for delivering care: Trajectory, Contingency, Chronicity models of care.
  • Anthropology of Chronic Diseases/Long Term Conditions: culture, health and illness, religion and spirituality, explanatory models of illness, body image and sexuality.
  • Self Management: Expert Patient Programme, Principles of empowerment.

What will I learn?

On successful completion of the module, you will be able to:

  1. Critically identify, apply, synthesise and evaluate relevant evidence relating to the principles of long term conditions that inform practice.
  2. Demonstrate advanced knowledge, skills and expertise in managing, holistically, the care of people with long term conditions in partnership with colleagues, families and carers.
  3. Analyse and critically appraise the frameworks/models that underpin long term conditions.
  4. Analyse critically the impact of culture and religion on the health of clients and the decision making processes utilised in accessing healthcare.
  5. Analyse and critically appraise the principles underpinning rehabilitation of clients suffering from long term conditions and provide critical insight in the management of clients with neurological, cardiac and pulmonary problems and those with recurring cancers.

Teaching and learning

The module will consist of a diversity of teaching and learning strategies, such as, taught lectures, group work, student-centred presentations and discussions through, web based learning (VLE and Blackboard) and input from outside speakers who are expert in their field. Students will be encouraged to share their previous experiences with their peers in order to enhance and maximise the learning experience.

The formative element of the assessment will facilitate the development of a case study regarding a patient with a rehabilitation issue.

Entry requirements

  • Registered professional qualification (e.g. nursing)
  • 60 level 6 credits (or equivalent)
  • Normally a minimum of 2 years experience in a relevant clinical area
  • Support of the clinical manager and a suitably qualified mentor/assessor
  • Opportunity to work outside usual clinical area, if necessary, to gain specific clinical competence
